Since there are many forms of self-employment, the documentation required may vary. Be sure to click the 'Self Employed' box when filling out your application, and our system will direct you to choose which category of self-employment applies to you.

|Independent Contractor||1099-MISC(s), for prior 2 years within the same field|
|Sole Proprietor||Federal Tax Returns (Form 1040) including Schedule C, for prior 2 years|
|Partnership||Federal Tax Returns (Form 1040) including Schedule E, for prior 2 years|
|S-Corp||Federal Tax Returns (Form 1040) including Schedule E, for prior 2 years|
|Corporation <25% ownership||Form 1040, 1099-MISC, 1099-DIV, and/or latest pay stub|
|Corporation >25% ownership||Form 1120, for prior 2 years|
