Since there are many forms of self-employment, the documentation required may vary. Be sure to click the 'Self Employed' box when filling out your application, and our system will direct you to choose which category of self-employment applies to you.

Sole Proprietor | Federal Tax Returns (Form 1040) including Schedule C, for prior 2 years |
Partnership | Federal Tax Returns (Form 1040) including Schedule E, for prior 2 years |
S-Corp | Federal Tax Returns (Form 1040) including Schedule E, for prior 2 years |
Corporation <25% ownership | Form 1040, 1099-MISC, 1099-DIV, and/or latest pay stub |
Corporation >25% ownership | Form 1120, for prior 2 years |
