- Address Confidentiality Program (ACP): If you are a participant in a state-run ACP, you can use the street address of the ACP’s sponsoring agency.
- Identification Number
- U.S. Individuals: Provide your Social Security Number (SSN) or a Taxpayer Identification Number (TIN).
- Non-U.S. Individuals: If you don’t have a Social Security number or TIN, provide your passport number and the country of issuance.
